FULL DESCRIPTIONS:

Our company is looking for the qualified Social Worker to start working urgently.

REQUIRED QUALIFICATIONS:

  • Must have Current SACSSP registration.
  • Must have professional indemnity insurance.
  • Must have minimum of 3 years’ social work experience.
  • Must have experience in hospital social work and case management.
  • Must have training and experience in palliative care.

REQUIRED SKILLS AND COMPETENCIES:  

  • Must be strong counselling, interpersonal and communication skills.
  • Must have strong report writing and time management skills.
  • Must have ability to work effectively and collaboratively within a multidisciplinary healthcare team.
  • Must have willingness to travel for outpatient consultations when required.

DUTIES AND RESPONSIBILITIES:

  • To conduct comprehensive psychosocial assessments and identify patients’ social, emotional and practical needs.
  • To provide counselling, emotional support and mediation to patients and their families.
  • To contribute to discharge planning and help facilitate safe and sustainable transitions of care.
  • To work collaboratively with the multidisciplinary team to coordinate patient care and rehabilitation goals.
  • To identify and address psychosocial barriers that may affect recovery, treatment adherence or successful discharge.
  • To facilitate referrals to appropriate healthcare, community and support services.
  • To advocate for patients and their families throughout the care journey.
  • To maintain accurate, professional and timely clinical documentation and reports.
  • To participate actively in multidisciplinary meetings and patient discussions.
  • To support patients and families navigating palliative care and complex healthcare circumstances.
  • To assist with outpatient consultations where required.
